I used to eat at Tokyo Kitchen twice a week for about 6 years. But one night not long ago I ordered shrimp tempura and I got severely sick.
I called the restaurant and they basically blew me off. I was pretty unhappy with their lack of interest in my complaint.
When I called the health department to report my illness they pointed me to their web site that lists the results of their inspections. All they would tell me is that this place consistently has the absolute lowest possible scores on their inspections.
I haven't been back since.
Their prices are a little higher than what you might expect for a 'fast food' type situation. They don't take orders at the table --- you queue up in line at the counter and pick-up your food when it's ready (they do occasionally deliver to the table).
The employees/owners are friendly and service is usually quite good. So I wish I could bring myself to go back but I probably won't.

Been eating here for years and have not gotten sick - best deal is the fried rice. Order ahead because the wait can be annoying.
